A photographer takes the images for the publification to the best of their ability. They give the model the brief and tell them what is expected of them, trying to ensure they get the shot that they need. There job is to take the brief from the publification and try to re-create what they want in the image.
Most people that go into photography either do A Levels or a National Diploma at college, where they can make a start to their portfolio, which is experience in its own right. From here you can then go onto further education to take a degree in photography. Photography mainly revolves around going out and taking pictures to put in your portfolio. You could also gain experience from working in a company and "following" the photographer before going off in your own right, as did Annie Leibovitz.
